12 MH-47D helicopters were produced. 6 were conversions from CH-47A models and 6 were conversions from CH-47C models. All aircraft were assigned to elements of the 160th Special Operations Aviation Regiment (SOAR) - "Nightstalkers", located at either Fort Campbell, Kentucky, or Hunter Army Airfield, near Savannah, Georgia.

Unfortunately, little is known about the individual aircraft and thier disposition.

As of December 2002, eleven MH-47D model airframes were in service to the United States Army.

By the end of 2009, there were no surviving MH-47D model airframes.
