At some point, 66-19066 was assigned to A Company - "Wildcats", 228th Assault Support Helicopter Battalion (ASHB), 1st Cavalry Division through at least 1968. On 24 April 1968, 66-19066 was transporting a sling load while on a resupply mission. While in the landing phase at approximately 200 feet above ground level, 66-19066 received small arms fire from the ground that damaged the aircraft and killed one crewmember - Door Gunner PFC Caballero. On 11 September 1985, 66-19066 was inducted into the D model program and converted to 85-24359. The last known location of 66-19066 was at Boeing during the conversion.
