             66-19084, Boeing build number B-342, was a CH-47A helicopter. The U.S. Army acceptance date was 18 April 1967. The administrative strike date was 8 November 1969. 66-19084 accumulated at least 1,540.0 aircraft hours.

   At some point, 66-19084 was assigned to the 228th Assault Support Helicopter Battalion (ASHB) and remained there until it was lost due to enemy fire.

   On 8 November 1969, 66-19084 sustained .30 and .50 caliber hits in the aft pylon. The sling load under transport was jettisoned and the aircraft crashed in flames from 50 feet. The crew and 12 passengers on board were uninjured.

   The last known location of 66-19084 was in the Republic of Vietnam.
