             68-15991, Boeing build number B-583, was a CH-47C helicopter. The U.S. Army acceptance date was 19 April 1969. 68-15991 accumulated 2,109.2 aircraft hours. At some point, 68-15991 was assigned to the 196th Aviation Company, 269th Aviation Battalion, 12th Aviation Group, 18th Corps, Forces Command (FORCOM), Fort Bragg, North Carolina, through at least 1977. On 29 May 1974, 68-15991 was involved in an accident. While at a cruise altitude of 1,000 feet and an airspeed 130 knots indicated, a loud bang noise was heard and the aircraft shuddered unusually with a winding down noise. There were no precautionary lights, or indications other than number 1 torque needle dropping to zero. The pilots elected to land and investigate. A visual inspection by the crew indicated the N2 section of the number 1 engine, serial number LE04513B, disintegrated in flight. The aircraft was equipped with T55-L7C engines. On 2 June 1977, 68-15991 was involved in an accident. 68-15991 was on a test flight to determine the airworthiness of the aircraft after replacing the differential collective pitch (DCP) actuator. At some point during the flight the number one engine lower cowling NSN 1560-00-103-5886 became unfastened and subsequently was ripped loose from its hinge and the aircraft. Negligible damage was done to the aircraft. On 20 January 1984, 68-15991 was inducted into the D model program and converted to 84-24152. The last known location of 68-15991 was at Boeing during the conversion. Aircraft status: Converted to D model.
