WHILE CRUISING AT 200 FT AGL, 130 KTS, PILOTS FELT LATERAL VIBRATIONS THAT CONTINUED TO INCREASE IN INTENSITY. ACFT WAS LANDED AND SHUT DOWN FOR INSPECTION. FORWARD ROTOR HEAD WAS TOO HOT TO TOUCH, TO TOUCH, HORIZONTAL HINGE PIN BEARING ON YELLOW BLADE HAD FAILED. HORIZONTAL HINGE PIN AND BEARING ON FORWARD ROTOR HEAD OVERHEATED.żż

Findings
DISASSEMBLY INSPECTION REPORT--CH-47C, 68-15993 CONCLUSIONS A. ANALYTICAL INVESTIGATION OF THE MAIN ROTOR HEAD ASSY REVEALED THE PRIMARY FAILURE WAS THROUGH SPALLING FATIGUE MECHANISMS OF THE PIN AND BEARING RACEWAYS. B. THE ROLLER BEARINGS FAILED THROUGH IMPACT LOADING. REF: LABORATORY REPORT NUMBER 78MX25 (INCL 11-18).
