92-00301, Boeing D model kit number M3442, was a CH-47D helicopter. The U.S. Army acceptance date was 25 October 1993. The administrative strike date was 16 April 2004. As of 31 December 2002, 92-00301 had accumulated 943.8 D model hours and 2,274.0 total aircraft hours. 92-00301 was a conversion from the original A model Chinook 61-02412. On 27 August 1992, 92-00301 was inducted into the D model program, converted, and scheduled for assignment to the 193rd Aviation Regiment, Army National Guard, located in the State of Hawaii. As of June 1995, 92-00301 was located at Fort Ruger, Hawaii, assigned to Company C - "Voyagers", 193rd Aviation, Army National Guard, United States Army Pacific (USARPAC). At some point in early 2004, 92-00301 deployed with the 193rd Aviation Regiment to the country of Iraq. On 14 April 2004, while in support of the Global War on Terrorism, 92-00301 was lost due to an accident approximately 70 nautical miles south-southwest of Baghdad. 92-00301 was one aircraft in a flight of three. While on an NVG mission the aircraft encountered a sandstorm. While attempting a precautionary landing, 92-00301 encountered brown-out conditions, crashed, rolled-over, and partially burned in the auxiliary power unit area. Of the crew of five, the two pilots and one crew member sustained minor injuries. All crew members were rescued by a combat search and rescue (CSAR) team some three hours after the accident. Due to the extensive damaged received during the crash sequence the airframe was deemed uneconomically repairable. After cannabalization of several usuable items, the United States Air Force was called in to destroy the wreakage with two 500 pound bombs. As of 1 January 2002, this aircraft was 38.9 years old.
