TRANSMITTED TO UNITS SUBORDINATE TO ADDRESSEES.  ADDRESSEES SHOULD

CLEARED WHEN THE INSPECTION OF PARA 8 BELOW IS COMPLETED.  THE
AFFECTED AIRCRAFT SHALL BE INSPECTED AS SOON AS PRACTICAL BUT NO
LATER THAN THE TASK/INSPECTION SUSPENSE DATE.  FAILURE TO COMPLY
WITH THE REQUIREMENTS OF THIS MESSAGE WITHIN THE TIME FRAME WILL
CAUSE THE STATUS SYMBOL TO BE UPGRADED TO A RED //X//.
    B.  AIRCRAFT IN DEPOT MAINTENANCE -  AIRCRAFT WILL NOT BE ISSUED
UNTIL COMPLIANCE WITH THIS ASAM HAS BEEN COMPLETED.

4.  SUMMARY OF PROBLEM -
    A.
         (1)  FIELD INSPECTIONS HAVE REVEALED IN SOME INSTANCES THAT
ROTARY WING HEAD BUSHINGS (P/N 145R3116-10) HAVE BECOME LOOSE.
THESE BUSHINGS ARE INSTALLED IN THE UPPER LAG DAMPER LUG OF THE
PITCH HOUSING.  LOOSE BUSHINGS CAN CAUSE EXCESSIVE WEAR IN THE LUGS
WHICH COULD RESULT IN HAVING TO SCRAP HIGH COST PITCH HOUSINGS.
         (2)  THE LOOSE BUSHINGS ARE THE RESULT OF DEFECTIVE
INSTALLATIONS.  INSTALLATIONS WERE PERFORMED BY RETROFIT PER MWO
1-1520-240-50-37 OR CONVERTED IN PRODUCTION OR AT OVERHAUL.
         (3)  THE NEW CONFIGURATION, MWO 1-1520-240-50-37, WAS
INSTITUTED BECAUSE FIELD REPORTS INDICATED THAT THE BUSHING P/N
114R2096-2 WAS SOMETIMES LEFT OUT OF THE LOWER LUG OF THE PITCH
HOUSING.  IF THE BUSHING P/N 114R2096-2 IS NOT PRESENT DURING
TORQUE-UP OF THE LAG DAMPER, IT COULD RESULT IN BENDING THE PITCH
HOUSING LUGS WHICH WOULD CAUSE SCRAPPING THE PART, AND PERHAPS
CRACKING OR BREAKING THE LUGS.
    B.  FOR MANPOWER/DOWNTIME AND FUNDING IMPACTS SEE PARA 12.
    C.  THE PURPOSE OF THIS MESSAGE IS TO INSPECT ALL AIRCRAFT WHICH
HAVE UPPER  BUSHINGS, P/N 145R3116-10, TO CORRECTLY REINSTALL ALL
UPPER BUSHINGS THAT HAVE SLIPPED, AND TO REQUIRE RECURRING
INSPECTIONS.  IF ANY BUSHINGS HAVE SLIPPED, ALL BUSHINGS IN THAT
ROTOR HEAD SHOULD BE INSPECTED.

PERFORM THE INSPECTION IN PARAGRAPHS (3) AND (4) SHALL BE PROVIDED
BY THE LOGISTICAL POINT OF CONTACT.  (ONE TOOL PER CHINOOK UNIT).
        (1)  VISUALLY INSPECT BUSHING 145R3116-10, THREE PLACES PER
HEAD, FOR SECURITY ON THE PITCH HOUSING/LAG DAMPER ASSEMBLY.
        (2)  IF ANY BUSHING ON A HEAD IS LOOSE OR DISPLACED ABOVE
THE TOP SURFACE OR BELOW THE BOTTOM SURFACE OF THE UPPER LUG,
PROCEED TO THE CORRECTION PROCEDURES IN PARA 9.
        (3)  PLACE THE TOOL P/N B39584 ON THE BOLT/BUSHING SHANK
BETWEEN BOLT/BUSHING HEAD P/N 145R3650-4 AND THE UPPER SURFACE OF
THE PITCH HOUSING/LAG DAMPER LUG AND CHECK FOR TOOL SNUGNESS.  THE
TOOL SHALL BE USED AS A GO-NO-GO GAGE.  NOT FIT
ON/AROUND THE BOLT/BUSHING SHANK, ASSUME THE BUSHING P/N 145R3116-10
IS LOOSE AND PROCEED TO THE CORRECTION PROCEDURES IN PARA 9.  IF THE
TOOL FITS SNUG, ASSUME THE BUSHING P/N 145R3116-10 IS SECURE AND
PROCEED TO THE NEXT PARA.
        (4)  PLACE THE TOOL P/N B39584 ON THE BOLT/BUSHING SHANK P/N
145R3650-4 BETWEEN THE LOWER SURFACE OF THE UPPER PITCH HOUSING /LAG
DAMPER LUG AND THE WASHER P/N 234R2205-1 (FOR ASSEMBLIES WITH TEFLON
LAG DAMPER BEARINGS) OR ON TOP OF THE BLADE LAG SHOCK ABSORBER P/N
114H6800 (FOR ASSEMBLIES WITHOUT WASHER P/N 234R2205-1) AND CHECK
FOR TOOL SNUGNESS.  THE TOOL SHALL BE USED AS A GO-NO-GO GAGE.  IF
TOOL DOES NOT FIT ON/AROUND THE BOLT/BUSHING SHANK P/N 145R3650-4,
PROCEED TO THE CORRECTION PROCEDURES IN PARA 9.  IF TOOL FITS SNUG,
THE INSPECTION IS COMPLETE.
                                 NOTE
    IF THE BUSHINGS HAVE SLIPPED, YOU HAVE 5 FLIGHT HOURS TO CORRECT
THE PROBLEM.
   B.  ANNOTATE DA FORM 2408-13-1 THAT REINSPECTION IS REQUIRED
AFTER THE 25TH, 50TH, AND 100TH FLIGHT HOUR.

    A.  REMOVE BLADES FROM THE PARTICULAR ROTARY WING HEAD ASSEMBLY
EFFECTED BY INSPECTION PROCEDURES IN PARA 8, TASK 5-64 OF REF 13.

145R3650-4 DURING DISASSEMBLY, REMOVE BUSHING P/N 145R3116-10 FROM
BOLT SHANK BY HAND FORCE ONLY.  IF BUSHING P/N 145R3116-10 DOES NOT
COME LOOSE BY HAND, CONTACT LOGISTICAL POINT OF CONTACT FOR
DISPOSITION INSTRUCTIONS, (REQUISITION REPLACEMENT PARTS).
    B.  REMOVE ALL BOLT ASSEMBLIES P/N 145R3650-4 FROM PITCH
HOUSING/LAG DAMPER ASSEMBLY IAW TM 55-1520-240-23, TASK 5-87.
                                 NOTE
IF BOLT/BUSHING ASSEMBLY P/N 145R3650-4 IS REPLACED FOR ANY OF THE
FOLLOWING REASONS, CONTACT LOGISTICAL POINT OF CONTACT FOR
DISPOSITION INSTRUCTIONS.
        (1)  VISUALLY INSPECT THE BOLT/BUSHING P/N 145R3650-4
ASSEMBLY FOR SERVICEABILITY.
        (2)  REPLACE ANY BOLT ASSEMBLIES P/N 145R3650-4 THAT ARE
FOUND TO HAVE SCORING OR OTHER DAMAGE ON THE BOLT SHANK OR ON THE
BUSHING OUTSIDE DIAMETER.
        (3)  IF BUSHING COMES DETACHED FROM BOLT ASSEMBLY, REPLACE
BOLT/BUSHING P/N 145R3650-4.
        (4)  MEASURE BOLT/BUSHING P/N 145R3650-4 OUTSIDE DIAMETER.
REPLACE BOLT/BUSHING IF OUTSIDE DIAMETER EXCEEDS 0.875 INCHES.
    C.  REMOVE ALL BUSHINGS P/N 145R3116-10 FROM THE PITCH HOUSING
ASSEMBLY.
                               WARNING
    WEAR PROTECTIVE GLOVES WHEN HANDLING HEATED PARTS.
                               CAUTION
    DO NOT COMPRESS OR SPREAD UPPER AND LOWER PITCH HOUSING LUGS.
         (1)  IF NECESSARY, HEAT THE ATTACHING LUG ON THE PITCH
HOUSING ASSEMBLY FOR A MAXIMUM OF 30 MINUTES USING A HEAT GUN.  THEN
REMOVE BUSHING P/N 145R3116-10 FROM THE PITCH HOUSING ASSEMBLY.
                               CAUTION
         DO NOT USE PETROLEUM BASED CLEANING SOLVENT.
                               WARNING
         ACETONE IS FLAMMABLE AND TOXIC. GOOD GENERAL VENTILATION IS
NORMALLY ADEQUATE.  SKIN AND EYE PROTECTION IS REQUIRED.
AVOID ALL SOURCES OF IGNITION.
         (2)  CLEAN BUSHING P/N 145R3116-10 USING A CLEAN CLOTH WET
WITH ACETONE (O-A-51) AND REMOVE ANY EXISTING ZINC CHROMATE PROMER
OR SEALING COMPOUND.
         (3)  INSPECT BUSHING P/N 145R3116-10 FOR SERVICEABILITY.
MEASURE OUTSIDE DIAMETER OF BUSHING.  IF BUSHING OUTSIDE DIAMETER IS
LESS THAN .9365 INCHES, DISCARD BUSHING AND REQUISITION REPLACEMENT
BUSHING.
                               CAUTION
         DO NOT USE PETROLEUM BASED CLEANING SOLVENT.
                               WARNING
         ACETONE IS FLAMMABLE AND TOXIC. GOOD GENERAL VENTILATION IS
NORMALLY ADEQUATE.  SKIN AND EYE PROTECTION IS REQUIRED.
AVOID ALL SOURCES OF IGNITION.
         (4)  USING A CLEAN CLOTH WET WITH ACETONE (O-A-51) TO CLEAN
UPPER LUG BUSHING BORES AND TO REMOVE ANY EXISTING ZINC CHROMATE
PRIMER OR SEALING COMPOUND.
         (5)  INSPECT THE UPPER LUG BORE IN THE PITCH HOUSING
ASSEMBLY FOR FRETTING AND CLEANLINESS.
         (6)  USING SCOTCH-BRITE WET WITH ACETONE, LIGHTLE PITCH HOUSING ASSEMBLY TO REMOVE
ANY DEBRIS OR FRETTING.
         (7)  TOUCH-UP REWORKED AREA WITH BRUSH ALODIZE (CHEMICAL
FILM COATING) IN ACCORDANCE WITH MIL-C-5541.
         (8)  MEASURE THE UPPER BUSHING BORE ON THE PITCH HOUSING
ASSEMBLY AT TWO POINTS 90 DEGREES APART.  ONE READING SHALL BE TAKEN
PARALLEL TO THE BLADE.  IF THE BORE EXCEEDS ITS MAXIMUM DIMENSION,
REJECT THE ROTARY WING HEAD ASSEMBLY.  THE MAXIMUM DIMENSION FOR THE
BORE IS 0.939 INCH MAXIMUM (POINT NINE-THREE-NINE).  REQUISITION
REPLACEMENT HEAD AND NOTIFY LOGISTICAL POINT OF CONTACT FOR ANY
REJECTED ROTARY WING HEAD ASSEMBLIES.
         (9)  CLEAN BUSHING P/N 145R3116-10 OUTSIDE DIAMETER AND
BOLT/BUSHING P/N 145R3650-4 USING A CLEAN CLOTH WET WITH ACETONE.
         (10)  USING A 145R3650-4 BOLT (SECURE THE PAWL IN A CLOSED
POSITION) INSTALL THE BUSHING P/N 145R3116-10 ON THE BOLT.
         (11)  RECLEAN OUTER SURFACE OF BUSHING P/N 145R3116-10
USING A CLEAN CLOTH WET WITH ACETONE.
                                 NOTE
         AIR DRY PRIMER/ACTIVATOR FOR 5 MINUTES PRIOR TO APPLICATION
OF LOCKING COMPOUND.
         (12)  COAT THE OUTER SURFACE OF THE BUSHING WITH
PRIMER/ACTIVATOR (MIL-S-22473) GRADE T, AND ALLOW TO AIR DRY.
                                 NOTE
         AIR DRY PRIMER/ACTIVATOR FOR 5 MINUTES PRIOR TO APPLICATION
OF LOCKING COMPOUND.
         (13)  COAT THE UPPER PITCH HOUSING ASSEMBLY P/N 145R2075
LUG BORE WITH PRIMER/ACTIVATOR (MIL-S-22473) GRADE T AND ALLOW TO
AIR DRY.
                               CAUTION
    THE UPPER LUG MUST BE AT ROOM TEMPERATURE WHEN THE LOC-TITE
    #635 IS APPLIED.
                                 NOTE
    ONCE ADHESIVE IS APPLIED OVER PRIMER ACTIVATOR, PARTS MUST BE
JOINED WITHIN 4 MINUTES.  IF THIS TIME IS EXCEEDED PARTS MUST BE
CLEANED AND RETREATED WITH PRIMER AND ADHESIVE.
         (14)  COAT THE UPPER LUG BORE OF THE PITCH HOUSING ASSEMBLY
P/N 145R2075 WITH LOC-TITE #635 (MIL-R-46082) RETAINING COMPOUND.
                                 NOTE
ENSURE O.D OF BUSHING SURFACE DOES NOT CONTAIN ANY CONTAMINATES WHEN
INSTALLING THE BOLT/BUSHING INTO THE LUG.  IF BUSHING DOES NOT
SEAT WITH HAND FORCE, LIGHTLY TAP THE HEAD OF THE BOLT WITH A
PLASTIC MALLET UNTIL BUSHING IS SEATED.  ENSURE BUSHING P/N
145R3116-10 IS NOT PROTRUDING BEYOND THE UPPER OR LOWER SURFACE OF
THE PITCH HOUSING LUG.
                               CAUTION
    DO NOT APPLY HEAT OR CHILL DURING INSTALLATION OF BUSHINGS.
         (15)  INSTALL BUSHING/BOLT INTO THE UPPER LUG BORE OF THE
PITCH HOUSING ASSEMBLY P/N 145R2075.  USE CAUTION TO ENSURE THAT THE
BUSHING P/N 145R3116-10 DOES NOT SEPARATE FROM BOLT DURING
INSTALLATION.
                                 NOTE
    PLACE A SIX INCH SCALE OR STRAIGHT EDGE ACROSS THE TOP FACE OF
THE UPPER LUG BORE ON THE PITCH HOUSING ASSEMBLY TO ENSURE THAT THE
BUSHING IS NOT PROTRUDING ABOVE THE SURFACE.  REPEAT THE ABOVE
INSTRUCTIONS FOR THE BOTTOM SURFACE.
         (16)  LEAVE THE BUSHING P/N 145R3116-10 IN THE PITCH
HOUSING ASSEMBLY P/N 145R2075 LUG FOR A MINIMUM OF 10 MINUTES BEFORE
REMOVING THE BOLT P/N 145R3650-4.
         (17)  IF AFTER DRYING EITHER TOP OR BOTTOM IS FOUND
PROTRUDING BEYOND THE SURFACE, REMOVE BUSHING AND REPEAT
INSTALLATION PROCEDURES IN ACCORDANCE WITH PARAGRAPH 9.C. THROUGH
PARAGRAPH 9.C.(16).
                               CAUTION
    LOC-TITE MATERIAL DOES NOT CURE SUFFICIENTLY AT TEMPERATURE
BELOW 40 DEGREES F.
         (18)  DO NOT SUBJECT JOINED PARTS TO HANDLING, VIBRATION OR
SHOCK UNTIL FULLY CURED.  ALLOW BUSHING INSTALLATION TO CURE FOR A

         (19)  CLEAN INSIDE DIAMETER OF BUSHING P/N 145R3116-10,
UPPER AND LOWER FACES OF THE PITCH HOUSING ASSEMBLY LUG, AND BOLT
