NOTE - THIS IS AN AVIATION SAFETY ACTION MESSAGE ISSUED PER AR 95-3,
CHAPTER 5 REVISION VIA MESSAGE HQ AVSCOM, AMSAV-XSOF, 181900Z SEP
90, SUBJECT:  CHANGE TO AR 95-3, CHAPTER 5, SAFETY OF FLIGHT
MESSAGES.  THIS MESSAGE HAS NOT BEEN TRANSMITTED TO UNITS
SUBORDINATE TO ADDRESSEES.  ADDRESSEES SHOULD IMMEDIATELY RETRANSMIT
THIS MESSAGE TO ALL SUBORDINATE UNITS, ACTIVITIES OR ELEMENTS
AFFECTED OR CONCERNED.  THE RETRANSMITTAL SHALL REFERENCE THE
MESSAGE.  ACTION ADDRESSES WILL IMMEDIATELY VERIFY THIS
RETRANSMISSION TO COMMANDER, ATCOM, ATTN - AMSAT-C-XS (SOF
COMPLIANCE OFFICER).

3.  REPORTING COMPLIANCE SUSPENSE DATE - REPORT COMPLIANCE IAW WITH
REQUIREMENTS OF CH-47-94-ASAM-01.
4.  SUMMARY OF PROBLEM -   THIS AMENDMENT TO THE BASIC ASAM IS
OFFERED TO AVOID UNNECESSARY DOWNTIME.  THE COMPLIANCE TIME IN THE
BASIC ASAM HAS BEEN EXTENDED UP TO 50 ADDITIONAL FLIGHT HOURS AFTER
FINDING A BUSHING WHICH HAS SLIPPED FROM ITS PROPER POSITION IAW
CH-47-94-ASAM-01.

6.  ASSEMBLY COMPONENTS TO BE INSPECTED -N/C.
7.  PARTS TO BE INSPECTED - N/C.
8.  INSPECTION PROCEDURES - PARA 8B AND PRECEDING NOTE IS AMENDED AS
FOLLOWS -
                                 NOTE
    IF THE BUSHINGS HAVE SLIPPED, YOU HAVE 50 FLIGHT HOURS TO
CORRECT THE PROBLEM.
   B.  ANNOTATE DA FORM 2408-13-1 THAT REINSPECTION IS REQUIRED
AFTER THE 25TH, 50TH, AND 100TH FLIGHT HOUR.  AFTER COMPLETION OF
THE 100TH FLIGHT HOUR AND NO BUSHINGS HAVE SLIPPED THE INSPECTION IS
COMPLETE. IF YOU HAVE ACCUMULATED 100 HRS SINCE ROTOR HEAD WAS
MODIFIED/PRODUCED OR OVERHAULED AND BUSHINGS HAVE NOT SLIPPED THE
INSPECTION IS COMPLETE.
         (1)  CH-47 UNITS SHOULD ENSURE THAT ALL ROTOR HEADS P/N
145R2003-9, AND 145R2004-18 HAVE BOLT BUSHING P/N 145R3650-4
INSTALLED IN THE INBOARD LAG DAMPER/ROTOR HEAD PITCH HOUSING BORE.
         (2)  SPARE ROTOR HEADS P/N 145R2003-9 AND 145R2004-18
RECEIVED FROM STOCK REQUIRE THE 25, 50 AND 100 HOUR REINSPECTION
AFTER INSTALLATION ON AIRCRAFT.
9.  CORRECTION PROCEDURES - CORRECTION PROCEDURES ARE AMENDED TO
INCLUDE -
    B.  IS AMENDED AT (2) AS FOLLOWS -
         (2)  FOR SCORING ON THE BOLTS SHANK -
              (A)  POLISH OUT MARKS ON THE BUSHING PART.
              (B)  SCORING MARKS ON THE SHANK ARE CAUSE FOR
REJECTION.
    C.  IS AMENDED AT (6) AND (14) TO INCLUDE PRECEEDING CAUTION AS
FOLLOWS -
         (6)  SCORING IN THE LUG BORE SHOULD NOT BE REMOVED BY
REAMING.  USING SCOTCH-BRITE WET WITH ACETONE, LIGHTLY POLISH THE
UPPER LUG BUSHING BORE OF THE PITCH HOUSING ASSEMBLY TO REMOVE
BURRS, SHARP EDGES AND ANY DEBRIS OR FRETTING.
 M TEMPERATURE WHEN THE LOC-TITE
    #635 OR LOCTITE #680 IS APPLIED.
         (14)  COAT THE UPPER LUG BORE OF THE PITCH HOUSING ASSEMBLY
P/N 145R2075 WITH LOC-TITE #635 (MIL-R-46082) OR LOCTITE #680
RETAINING COMPOUND.
