SEP 90, SUBJECT:  CHANGE TO AR 95-3, CHAPTER 5, SAFETY OF FLIGHT
MESSAGES.  THIS MESSAGE HAS NOT BEEN TRANSMITTED TO UNITS
SUBORDINATE TO ADDRESSEES.  ADDRESSEES SHOULD IMMEDIATELY

HORIZONTAL RED //-//.  THE AFFECTED AIRCRAFT SHALL BE INSPECTED AS
SOON AS PRACTICAL BUT NO LATER THAN THE TASK/INSPECTION SUSPENSE
DATE.  FAILURE TO COMPLY WITH THE REQUIREMENTS OF THIS MESSAGE
WITHIN THE TIME FRAME WILL CAUSE THE STATUS SYMBOL TO BE UPGRADED
TO A RED //X//.  WHILE THE AIRCRAFT IS ON A HORIZONTAL RED //-// IT
MAY BE REPORTED AS FULLY MISSION CAPABLE (FMC).

2.  TASK/INSPECTION SUSPENSE DATE - WITHIN 10 HOURS/14 DAYS.
3.  REPORTING COMPLIANCE SUSPENSE DATE - REPORT COMPLIANCE TO
COMMANDER, ATCOM, ATTN:  AMSAT-C-XS, NO LATER THAN 8 FEB 94 PER
PARA 14A OF THIS MESSAGE.
4.  SUMMARY OF PROBLEM -
    A.  DURING GROUND RUN-UP, A CONNECTING LINK, P/N 145C3340-10
WAS BROKEN.  TEARDOWN ANALYSIS REVEALED THAT THE TENSILE STRENGTH
OF THE CONNECTING LINK WAS BELOW DRAWING REQUIREMENTS.  ANALYSIS
ALSO REVEALED A 3 INCH LENGTHWISE SCRAPE MARK ON THE FAILED LINK
WHICH CONTRIBUTED TO ITS FAILURE.  THIS SCRAPE MARK WAS ATTRIBUTED
TO A FOREIGN OBJECT BECOMING LODGED BETWEEN THE LINK AND AN
ADJACENT BULKHEAD.
    B.  THE LOW STRENGTH WAS A RESULT OF FAULTY HEAT TREAT ON
APPROXIMATELY 148 CONNECTING LINKS, P/N 145C3340-10 WITH A
MANUFACTURING DATE OF "1-2-91" AND THE WORD "GLOBE" INK STAMPED ON
THE LINK.  THEREFORE, CONNECTING LINKS IN THIS LOT REQUIRE A
RECURRING CONTROLS CHECK PRIOR TO EACH ENGINE START UNTIL THEY ARE
REPLACED.  REPLACEMENT CONNECTING LINKS WILL BE PROVIDED TO THE
UNITS AS FREE ISSUE.  REPRESENTATIVE SAMPLES OF OTHER LOTS WILL BE
REQUIRED FOR TESTING.  SERVICEABLE CONNECTING LINKS, P/N
145C3340-10 WILL BE FURNISHED (SUBJECT TO AVAILABILITY) BY
LOGISTICAL POC.  THE INSPECTION VARIES DEPENDING ON AIRCRAFT SERIAL
NUMBER.  SEE PARA 8 FOR INSPECTION PROCEDURES.  ALL UNITS SHALL
REPORT TO LOGISTICAL POC WITHIN 10 DAYS FROM THE DATE OF THIS
MESSAGE PER PARA 14B.   FUTURE PROCUREMENTS OF CONNECTING LINKS
WILL BE SERIALIZED AND/OR CONTAIN METALCALS.
    C.  PILOTS INFORMATION - THESE LINKS ARE SUBJECT TO LOW FORCES
DURING NORMAL FLIGHT.  THE CONDITIONS OF CONCERN ARE:
       (1) BLADE FLAPPING WHILE PARKED (HYDRAULICS OFF).
       (2) PILOT INDUCED FORCE TO OVERCOME A JAM.
       (3) IMPROPER SHUTDOWN PROCEDURE (THRUST NOT IN DETENT).
THE FOLLOWING CHANGE WILL BE MADE TO TM 55-1520-240-10, PARAGRAPH
8-42, SUBPARAGRAPH 1 (ENGINE SHUTDOWN).
                                CAUTION
    CRITICAL FLIGHT CONTROL COMPONENTS CAN BE DAMAGED IF THRUST IS
    NOT IN GROUND DETENT.
    D.  THE PURPOSE OF THIS MESSAGE IS TO:
         (1)  DIRECT A ONE TIME INSPECTION TO DETERMINE IF
CONNECTING LINK, P/N 145C3340-10 IN THE SUBJECT LOT ARE INSTALLED.
         (2)  DIRECT A RECURRING CONTROLS CHECK, PRIOR TO EACH
ENGINE START, OF CONNECTING LINK, P/N 145C3340-10 IN THE SUBJECT
LOT UNTIL REPLACEMENT.
         (3)  DIRECT REPLACEMENT OF CONNECTING LINK, P/N
145C3340-10 IN SUSPECT LOT WHEN REPLACEMENT LINKS ARE AVAILABLE.
         (4)  INFORM PILOTS OF CONDITIONS THAT SUBJECT LINKS TO
DAMAGING FORCES AND RELATED CHANGE TO TM 55-1520-240-10.  INSERT A
COPY OF THIS MESSAGE IN THE PILOTS READING FILE.
5.  END ITEMS TO BE INSPECTED - ALL MH-47D, CH-47D AND MH-47E
AIRCRAFT.  SERIAL NUMBERS 92-00294 AND SUBSEQUENT WILL BE
INSPECTED/CORRECTED BY CONTRACTOR DURING PRODUCTION AND THEREFORE
RECORDS FOR AIRCRAFT SERIAL NUMBERS 92-00294 AND SUBSEQUENT WILL
NOT REQUIRE IDENTIFICATION OF THIS ACTION.

CONSIDERED SERVICEABLE.  THESE LINKS DO NOT REQUIRE THE RECURRING
CONTROLS CHECK OR REPLACEMENT AND THE INSPECTION IS COMPLETE.
                             NOTE
SOME CONNECTING LINKS WILL HAVE A PART MARKING "145C3339-9" INK
STAMPED ON THE TUBE.  DO NOT MISTAKE THIS PART MARKING WITH THE
ASSEMBLY PART MARKING OF 145C3340-10.  THE TUBE IS PROCURED
SEPARATELY FROM THE ROD ENDS, THEREFORE, WHEN THE ROD ENDS ARE
ASSEMBLED ONTO THE TUBE, THE PART NUMBER CHANGES TO "145C3340-10".
    A.  DETERMINE MANUFACTURER'S PART MARKING ON THE CONNECTING
LINK, P/N 145C3340-10.
    B.  DETERMINE AIRCRAFT SERIAL NUMBER AND PERFORM ONE OF THE
FOLLOWING INSPECTIONS BASED ON THE AIRCRAFT SERIAL NUMBER:
         (1)  PERFORM THE FOLLOWING INSPECTION FOR AIRCRAFT WITH
S/N 90-00189 THROUGH S/N 92-00293:
              (A)  ALL CONNECTING LINKS P/N 145C3340-10 THAT HAVE
THE WORD "GLOBE", OR THE DATE OF "1-2-91", OR HAVE NO IDENTIFIABLE
MARKINGS ON THE LINK SHALL BE CONSIDERED SUSPECT.  PERFORM THE
RECURRING CONTROLS CHECK AND REPLACE CONNECTING LINK PER PARAGRAPH
9.  IF IN DOUBT, CONTACT TECHNICAL OR LOGISTICAL POC.
              (B)  ALL OTHER CONNECTING LINKS P/N 145C3340-10 THAT
DO NOT MEET THE CRITERIA IN THE ABOVE PARAGRAPH 8.B.(1)(A), ARE
CONSIDERED SERVICEABLE AND THE INSPECTION IS COMPLETE.
                                NOTE
CONNECTING LINKS MANUFACTURED AFTER 1-2-91 HAVE BEEN INSPECTED AND
TESTED AT BOEING HELICOPTERS.  THE RESULTS INDICATE THAT ALL
CONNECTING LINKS MANUFACTURED AFTER 1-2-91 MEET SPECIFICATIONS.
         (2)  PERFORM THE FOLLOWING INSPECTION FOR AIRCRAFT WITH
S/N 81-23381 THROUGH 90-00188:
                                NOTE
THERE IS NO EVIDENCE THAT THE SUBJECT LOT OF 148 CONNECTING LINKS
WERE INSTALLED ON THESE AIRCRAFT.  HOWEVER, TO ENSURE THAT ONE OF
THESE LINKS IS NOT ON AN AIRCRAFT AND TO ENSURE THAT THERE IS NOT A
FLEET-WIDE PROBLEM, A REPRESENTATIVE SAMPLE OF OTHER LOTS BASED ON
AIRCRAFT SERIAL NUMBER IS REQUIRED.
                                NOTE
A SAMPLING PLAN HAS BEEN ESTABLISHED IN WHICH SPECIFIC CONTROL RODS
DESIGNATED BY AIRCRAFT SERIAL NUMBER WILL BE REQUIRED FOR TESTING.
THE LOGISTICAL POC WILL CALL THE SELECTED UNITS.  IF SELECTED, THE
CONNECTING LINK SHALL BE REPLACED PER PARAGRAPH 9.B.  EVERYTHING IN
PARAGRAPH 9 SHALL BE ADHERED TO WITH THE EXCEPTION OF PERFORMING
THE CONTROLS CHECK.  A CONTROLS CHECK WILL NOT BE REQUIRED FOR
AIRCRAFT TESTED UNDER THIS SAMPLING PLAN.
              (A)  IF PART IS INK STAMPED WITH A MANUFACTURING DATE
OF "1-2-91", PERFORM THE RECURRING CONTROLS CHECK AND REPLACE
CONNECTING LINK, P/N 145C3340-10 PER PARAGRAPH 9.
              (B)  IF AN AIRCRAFT IS NOT PART OF THE SAMPLE TESTING
AND THE PART DOES NOT HAVE THE MANUFACTURING DATE OF "1-2-91", THE
PART IS CONSIDERED SERVICEABLE AND THE INSPECTION IS COMPLETE.
         (3)  FOR AIRCRAFT WITH S/N 92-00294 AND SUBSEQUENT, NO
ACTION IS REQUIRED.  THE INSPECTION IS CONSIDERED COMPLETE.
9.  CORRECTION PROCEDURES -
    A.  CONTROLS CHECK - PRIOR TO EACH ENGINE START DURING THE
ENGINE START-UP PROCEDURE, VISUALLY INSPECT THE CONNECTING LINK,
P/N 145C3340-10 FOR SERVICEABILITY.  WITH THE APU POWER APPLIED,
PERFORM FLIGHT CONTROL TRAVEL AND HYDRAULIC CHECK PER PARAGRAPH
8-23, SUBPARAGRAPH 21, OF TM 55-1520-240-10.  VISUALLY OBSERVE THE
CONNECTING LINK, P/N 145C3340-10 FOR INTERFERENCE WITH SURROUNDING
STRUCTURE OR EVIDENCE OF BENDING.  IF THE LINK CONTACTS THE
STRUCTURE OR APPEARS DEFORMED, THE LINK MUST BE REPLACED PRIOR TO
FLIGHT.  IF FOR ANY REASON, THE START-UP PROCEDURES ARE ABORTED
DURING THE INSPECTION, REPEAT THE ABOVE FLIGHT CONTROL TRAVEL AND
HYDRAULIC CHECK.
    B.  REPLACEMENT -

CONNECTING LINK, P/N 145C3340-10.  IF THE LINK DOES NOT HAVE ANY
MARKINGS, INDICATE "NO MARKINGS".  IF THE LINK HAS A METALCAL
AFFIXED, INDICATE "METALCAL".  REPORTS SHALL BE COMPLETED NO LATER
THAN 10 DAYS FROM THE DATE OF THIS MESSAGE.
