CHAPTER 5 REVISION VIA MESSAGE HQ AVSCOM, AMSAV-XSOF, 181900Z SEP 90,
SUBJECT:  CHANGE TO AR 95-3, CHAPTER 5, SAFETY OF FLIGHT MESSAGES.
THIS MESSAGE HAS NOT BEEN TRANSMITTED TO UNITS SUBORDINATE TO
ADDRESSEES.  ADDRESSEES SHOULD IMMEDIATELY RETRANSMIT THIS MESSAGE TO
ALL SUBORDINATE UNITS, ACTIVITIES OR ELEMENTS AFFECTED OR CONCERNED.

HORIZONTAL DASH //-//.  THE RED HORIZONTAL DASH //-// MAY BE CLEARED
WHEN THE INSPECTION OF PARA 8 BELOW IS COMPLETED.  THE AFFECTED
AIRCRAFT SHALL BE INSPECTED AS SOON AS PRACTICAL BUT NO LATER THAN
THE  TASK/INSPECTION SUSPENSE DATE.  FAILURE TO COMPLY WITH THE
REQUIREMENTS OF THIS MESSAGE WITHIN THE TIME FRAME WILL CAUSE THE
STATUS SYMBOL TO BE UPGRADED TO A CIRCLED RED //X// FOR THE 5000
POUND  TIE-DOWN RECEPTACLES ONLY.
    B.  AIRCRAFT IN DEPOT MAINTENANCE - SAME AS PARA 1A. AIRCRAFT
WILL  NOT BE ISSUED UNTIL COMPLIANCE WITH THIS ASAM HAS BEEN
COMPLETED.

AIRCRAFT WILL NOT BE RELEASED UNTIL COMPLIANCE WITH THIS ASAM

PERFORM INSPECTION PRIOR TO INSTALLATION IN AIRCRAFT.  TM CHANGES

4.  SUMMARY OF PROBLEM -
     A.  TWO INCIDENTS HAVE BEEN REPORTED OF CREW MEMBERS SAFETY
HARNESS BECOMING UNSECURED FROM THE 5000 POUND TIE-DOWN RECEPTACLE
P/N  114S2893 WHEN THE TIEDOWN ADAPTER BECAME UNSCREWED FROM THE
TIE-DOWN FITTING.  UPON LANDING, THE TIE-DOWN FITTING AND ADAPTER
WERE VISUALLY INSPECTED, AND IT WAS FOUND THAT THE TIE DOWN ADAPTER
HAD UNSCREWED FROM THE RECEPTACLE RETAINING BUSHING.  THERE WAS NO
EVIDENCE OF SEALANT ON THE ADAPTER STUD THREADS WHICH IS REQUIRED TO
KEEP THE FITTING FROM UNSCREWING.  ADDITIONAL INVESTIGATION
REVEALED THAT THE EXACT CAUSE OF THE PROBLEM WAS MISSING MIL-S-
22473 SEALANT COATING ON THE RETAINING BUSHING THREADS, AND SEIZURE
OF  THE BUSHING DUE TO PAINT OVERSPRAY, ALLOWING THE TIE-DOWN RING TO
UNSCREW ON ROTATION.
     B.  THE PURPOSE OF THIS MESSAGE IS TO DIRECT A ONE TIME
INSPECTION OF THE 5000 POUND TIE-DOWN RECEPTACLE ASSEMBLIES AND
PROVIDE INSTRUCTIONS TO CORRECT DEFECTIVE TIE-DOWN ASSEMBLIES.
5.  END ITEMS TO BE INSPECTED - ALL MH-47D, CH-47D AND MH-47E
AIRCRAFT.  SERIAL NUMBERS 81-23381 THROUGH SUBSEQUENT. AIRCRAFT
SERIAL  NUMBERS 92-00308 AND SUBSEQUENT WILL BE INSPECTED/CORRECTED
BY  CONTRACTOR DURING PRODUCTION AND THEREFORE RECORDS FOR AIRCRAFT
SERIAL NUMBERS 92-00308 AND SUBSEQUENT WILL NOT REQUIRE
IDENTIFICATION  OF THIS ACTION.

INSPECT THE 5000 POUND TIEDOWN RING ASSEMBLY IN CABIN FLOOR AND RAMP
FOR SERVICEABILITY AS FOLLOWS:
    A.  ROTATE THE RING ASSEMBLY BY HAND IN BOTH DIRECTIONS.
    B.  IF RING CANNOT BE ROTATED BY HAND, THE ASSEMBLY IS SUSPECT
AND  MUST BE REPAIRED OR REPLACED.
    C.  ATTACH A SPRING SCALE TO THE TIEDOWN RING AND APPLY 5 POUNDS
OF FORCE IN AN UPWARD DIRECTION.  WHILE MAINTAINING THE TENSION ON
THE  SCALE, ROTATE THE RING ASSEMBLY 10 COUNTER-CLOCKWISE TURNS.  IF
THE RING STARTS TO UNSCREW, OR SCREWS OUT OF THE RETAINING BUSHING,
PROCEED TO PARA 9 FOR THE CORRECTIVE PROCEDURES.
9.  CORRECTION PROCEDURES -
                            NOTE
THE 5000 POUND TIE-DOWN ASSEMBLIES ARE ATTACHED TO RAIL ASSEMBLIES.
THE TIE-DOWN ASSEMBLIES CONSIST OF A RECEPTACLE, RING ADAPTER, AND
BUSHING.  THE ASSEMBLY IS ATTACHED TO BEAM ASSEMBLIES BY RIVETING THE
RECEPTACLE TO THE BEAM.  THE BEAM IS SECURED TO THE AIRCRAFT MAIN
FLOOR BEAMS BY FOUR SCREWS IN THE BOTTOM OF EACH RECEPTACLE.
    A.  IF SUSPECT RING ASSEMBLIES ARE LOCATED ON THE LEFT OR RIGHT
OUTBOARD BL 44 BEAMS PROCEED AS FOLLOWS:
        (1)  REMOVE FLOOR PANELS ADJACENT TO THE SUSPECT RING.
                           CAUTION
TO PREVENT DAMAGE TO THE FLOOR SUPPORT BEAM, INSERT A WOODEN BLOCK,
OR  A PIECE OF PHENOLIC BETWEEN THE RECEPTACLE AND THE FLOOR SUPPORT
BEAM WHEN DRILLING OUT RECEPTACLE RIVETS.
        (2)  DRILL OUT RIVETS ATTACHING THE RECEPTACLE TO THE
BEAM ASSEMBLY.
        (3)  REMOVE THE TIE-DOWN ASSEMBLY FROM THE BEAM.
        (4)  SECURE THE BUSHING WITH A SUITABLE TOOL AND REMOVE
TIE-DOWN RING AND BUSHING FROM THE RECEPTACLE.
                           WARNING
METHYL-ETHL-KETONE IS FLAMMABLE AND TOXIC.  IT CAN IRRITATE SKIN AND
CAUSE BURNS.  USE ONLY WITH ADEQUATE VENTILATION, AWAY FROM HEAT OR
OPEN FLAME.  AVOID CONTACT WITH SKIN, EYES, OR CLOTHING.  IN CASE OF
CONTACT, IMMEDIATELY FLUSH SKIN OR EYES WITH WATER FOR AT LEAST 15
MINUTES.  GET MEDICAL ATTENTION FOR EYES.
        (5)  CLEAN THE RING ADAPTER THREADS, AND BUSHING THREADS
USING  A WIRE BRUSH AND CLEANING SOLVENT (METHYL-ETHYL-KETONE).
                           WARNING
SEALANT MIL-S-22473 IS FLAMMABLE AND TOXIC.  IT CAN IRRITATE SKIN AND
CAUSE BURNS.  USE ONLY IN A WELL VENTILATED AREA, AWAY FROM HEAT AND
OPEN FLAME.  IN CASE OF CONTACT, IMMEDIATELY FLUSH SKIN OR EYES WITH
WATER FOR AT LEAST 15 MINUTES.  GET MEDICAL ATTENTION FOR EYES.
        (6)  COAT THE THREADS OF THE ADAPTER AND BUSHING WITH
PRIMER ACTIVATOR MIL-S-22473 GRADE T, USING A BRUSH (MIL-B-23958).
WEAR GLOVES (MIL-G-10902).  ALLOW THE PRIMER ACTIVATOR TO AIR DRY FOR
A MINIMUM OF FIVE MINUTES PRIOR TO APPLICATION OF SEALANT.
        (7)  COAT THE THREADS OF THE ADAPTER AND BUSHING WITH
SEALANT MIL-S-22473 USING A BRUSH (MIL-B-23958).  WEAR GLOVES
(MIL-G-10902).
        (8)  REINSTALL THE TIE-DOWN RECEPTACLE ASSEMBLY PER TASK
2-206, STEPS 2 THROUGH 5.  ENSURE THAT THE BUSHING IS COMPLETELY
THREADED ONTO THE RING ADAPTER.  LOCKING SURFACES SHALL BE COVERED
WITH SEALANT AS EVIDENCED BY A CONTINUOUS FILLET AT THE PARTING LINE
OF THE BUSHING AND ADAPTER.  REMOVE EXCESS SEALANT.  ALLOW THE
SEALANT  TO CURE FOR ONE HOUR AT 70 DEGREES F OR ABOVE AND 24 HOURS
AT  40-70 DEGREES F.
        (9)  REINSTALL THE CABIN FLOOR PANEL PER TASK 2-207.
    B.  IF SUSPECT RING ASSEMBLIES ARE LOCATED ON THE INBOARD
BEAM ASSEMBLIES PROCEED AS FOLLOWS:
        (1)  REMOVE FLOOR PANELS ON THE LEFT AND RIGHT HAND SIDES OF
THE ENTIRE LENGTH OF THE TIE-DOWN BEAM ASSEMBLIES.
        (2)  REMOVE THE FOUR SCREWS AT THE BOTTOM OF EACH OF THE
TIE-DOWN RECEPTACLES, AND THE SCREWS AT EACH ATTACHING END OF THE
BEAM  ASSEMBLY.
        (3)  REMOVE THE BEAM ASSEMBLY FROM THE AIRCRAFT.
        (4)  SECURE THE BUSHING WITH A SUITABLE TOOL AND REMOVE
TIE-DOWN RING AND BUSHING FROM THE RECEPTACLE.

2-206, STEPS 2 THROUGH 5.  ENSURE THAT THE BUSHING IS COMPLETELY
THREADED ONTO THE RING ADAPTER.  LOCKING SURFACES SHALL BE COVERED
WITH SEALANT AS EVIDENCED BY A CONTINUOUS FILLET AT THE PARTING LINE
OF THE BUSHING AND ADAPTER.  REMOVE EXCESS SEALANT.  ALLOW THE
SEALANT TO CURE FOR ONE HOUR AT 70 DEGREES F OR ABOVE AND 24 HOURS AT
40-70 DEGREES F.
        (9)  REINSTALL THE BEAM ASSEMBLIES IN THE AIRCRAFT.
        (10)  REINSTALL THE CABIN FLOOR PANELS PER TASK 2-207.
    C.  IF SUSPECT RING ASSEMBLIES ARE LOCATED ON THE RAMP,
PROCEED AS FOLLOWS:
        (1)  REMOVE THE AFFECTED RAMP FLOOR PANEL PER TASK 2-247.
        (2)  SECURE THE BUSHING WITH A SUITABLE TOOL AND REMOVE
       TIE-DOWN RING AND BUSHING FROM THE RECEPTACLE.
