PROVIDED NO ALTERNATE INSTRUCTIONS TO DOCUMENT THE PERFORMANCE
OF PHASE, PERIODIC, PMP AND PROGRESSIVE PHASE MAINTENANCE.
SINCE THEN, THIS COMMAND HAS RECEIVED NUMEROUS DA FORMS 2028
REQUESTING DETAILED INSTRUCTIONS FOR DOCUMENTING THESE
INSPECTIONS.
    B.  TO ELIMINATE CONFUSION, THE FOLLOWING INSTRUCTIONS WILL

    C.  THE PURPOSE OF THIS MESSAGE IS TO PROVIDE FORMS AND
RECORDS INSTRUCTIONS TO DOCUMENT PERFORMANCE OF PHASE, PERIODIC,
PMP, AND PROGRESSIVE PHASE MAINTENANCE INSPECTIONS.

    A.  THE MAINTENANCE INSPECTION CHECKLIST HAS TWO MAJOR
PURPOSES:
       (1)  TO PROVIDE SCHEDULED MAINTENANCE INSPECTION
REQUIREMENTS TASKS; WHEN THE TASKS WILL BE ACCOMPLISHED AND WHAT
CONDITIONS ARE DESIRED.
       (2)  TO RECORD THE INITIALS OR PID AND THE MAN-HOURS OF
THE PERSON THAT ACCOMPLISHES EACH INSPECTION REQUIREMENT TASK.
    B.  INSPECTION CHECKLISTS WILL BE USED BY AVUM MAINTENANCE
ACTIVITIES, WITH ASSISTANCE OF AVIM AND DEPOT LEVEL
MAINTENANCE ACTIVITIES, FOR THE ACCOMPLISHMENT OF SCHEDULED
MAINTENANCE INSPECTIONS (I.E., INTERMEDIATE, PHASE, PERIODIC,
COMBAT PHASE OR PROGRESSIVE PHASED MAINTENANCE).  INSPECTION
CHECKLISTS WILL NOT CONTAIN SPECIAL INSPECTIONS, REPLACEMENT OF
REPAIR PARTS, SERVICES, LUBRICATION ORDERS, ETC., THAT ARE
LISTED IN THE SPECIAL INSPECTIONS SECTION  OF THE APPLICABLE TM
-23 MAINTENANCE MANUAL AND ARE RECORDED ON DA FORM 2408-18
(EQUIPMENT INSPECTION LIST).  CHECKLISTS WILL NOT CONTAIN
INSTRUCTIONS FOR REPAIR, ADJUSTMENT, OR OTHER MEANS OF
RECTIFYING CONDITIONS, NOR WILL THEY CONTAIN INSTRUCTIONS FOR
TROUBLESHOOTING TO FIND CAUSES FOR MALFUNCTIONING.  THE
INSTRUCTIONS FOR REPAIR, ETC., CAN BE FOUND IN THE TM -23
MAINTENANCE MANUAL THAT APPLIES.
    C.  ALL MAINTENANCE INSPECTION CHECKLISTS CURRENTLY IN USE
WILL BE REVISED TO BE COMPATIBLE WITH THE NEWLY DESIGNED FORMS
AND RECORDS CONTAINED IN DA PAMPHLET 738-751.  UNTIL THE
INSPECTION CHECKLISTS ARE CHANGED THE FOLLOWING INSTRUCTIONS
WILL BE FOLLOWED:
       (1)  A NEW INSPECTION CHECKLIST WILL BE USED EACH TIME AN
AIRCRAFT UNDERGOES AN INTERMEDIATE, PHASE, PERIODIC, COMBAT
PHASE OR PROGRESSIVE PHASED MAINTENANCE INSPECTION.
       (2)  THE CHECKLIST IS RELATED TO THE DA FORM
2408-13-1/2408-13-1-E, LOCATED IN THE AIRCRAFT LOGBOOK, WHICH IS
USED TO RECORD ANY FAULTS FOUND DURING THE INSPECTION AND THE
COMPLETION OF THE SCHEDULED MAINTENANCE INSPECTION.  FAULTS,
DEFICIENCIES, AND INSPECTIONS WILL NOT BE ENTERED IN THE
CHECKLIST.
       (3)  THE CHECKLIST MAY BE SEPARATED FOR DISTRIBUTION TO
THE MAINTENANCE CREW, INCLUDING ALLIED SHOPS.  DO NOT RE-ENTER
FAULTS, REMARKS OR DEFICIENCIES FROM THE DA FORM
2408-13-1/2408-13-1-E/2408-13-3/2408-14/2408-14-E AND SPECIAL
INSPECTIONS, SERVICES, OR  REPLACEMENTS RECORDED ON DA FORM
2408-18/2408-18-E TO THE INSPECTION CHECKLIST.
       (4)  ALL FAULTS AND DEFICIENCIES DISCOVERED DURING THE
INSPECTION WILL BE RECORDED ON DA FORM 2408-13-1/2408-13-1-E AND
RELATED MAINTENANCE ACTIONS TO CORRECT THESE FAULTS OR
DEFICIENCIES WILL BE RECORDED ON DA FORM 2408-13-2/2408-13-2-E
PER CHAPTER 2 OF DA PAMPHLET 738-751.  ALL FAULTS AND
DEFICIENCIES WILL BE PRECEDED BY THE INSPECTION NUMBER AND
INSPECTION REQUIREMENT TASK NUMBER IN BLOCK 7 OF THE DA FORM
2408-13-2/2408-13-2-E.
       (5)  WHEN A TECHNICAL INSPECTION IS PERFORMED BEFORE,
DURING, OR AT THE COMPLETION OF A SCHEDULED MAINTENANCE
INSPECTION, ON AIRCRAFT AND AVIATION ASSOCIATED EQUIPMENT (I.E.,
REPAIR PARTS OR COMPONENTS/MODULES) DA FORM 2408-13-3 WILL BE
USED TO RECORD THE RESULTS.  WHEN RELATED MAINTENANCE ACTIONS
ARE REQUIRED TO CORRECT FAULTS AND DEFICIENCIES RECORDED ON THE
DA FORM 2408-13-3, USE DA FORM 2408-13-2/2408-13-2-E PER CHAPTER
2, OF DA PAMPHLET 738-751.  DA FORMS 2408-13-3 COMPLETED DURING
THE SCHEDULED MAINTENANCE INSPECTION WILL BECOME PART OF THE
LOGBOOK FLIGHT PACK.
       (6)  DURING THE ACCOMPLISHMENT OF A SCHEDULED INSPECTION,
IT MAY BE NECESSARY TO REMOVE A REPAIR PART, COMPONENT,
ASSEMBLY, ETC., FOR REPAIR, INSPECTION, SERVICE OR OTHER
MAINTENANCE.  THE ITEM IS THEN SENT TO THE SUPPORTING AVIM UNIT
OR THE COLLOCATED ALLIED SHOPS, IN SOME UNITS, FOR THE
MAINTENANCE ACTION.  WHEN THIS OCCURS, A DA FORM 2407
(MAINTENANCE REQUEST) IS NORMALLY USED FOR THE WORK NEEDED, THEN
PASSED ON TO THE AVIM TO DO THE WORK.  A DA FORM 2408-13-3 WILL
BE INITIATED BY THE AVIM LEVEL TECHNICAL INSPECTOR TO PERFORM
THE INITIAL INSPECTION ON THE REPAIR PART, COMPONENT, MODULE,
ETC.  THE DA FORM 2408-13-3 WILL ACCOMPANY THE ITEM TO THE SHOP
FOR CORRECTIVE ACTION OF THE FAULT OR COMPLETION OF THE
MAINTENANCE ACTION.  WHEN RELATED MAINTENANCE ACTIONS ARE NEEDED
TO CORRECT THE FAULT OR COMPLETE THE MAINTENANCE ACTION, A DA
FORM 2408-13-2/2408-13-2-E WILL BE USED.
       (7)  UPON COMPLETION OF THE SCHEDULED MAINTENANCE
INSPECTION AND BEFORE REQUIRED TEST FLIGHT IS ACCOMPLISHED,
ASSEMBLE ALL LOGBOOK, MAINTENANCE, HISTORICAL FORMS AND RECORDS,
INCLUDING THE INSPECTION CHECKLIST THAT WAS ANNOTATED DURING THE
SCHEDULED INSPECTION.  ACCOMPLISH A FORMS AND RECORDS CHECK,
USING THE MAINTENANCE RECORDS CHECKLIST IN TABLE 3-2, PAGE 87,
TO ASSURE THAT PROPER AND ACCURATE ENTRIES WERE MADE AND ALL
FORMS AND RECORDS THAT APPLY ARE PRESENT, COMPLETED AND UPDATED
BEFORE RELEASING THE AIRCRAFT FROM MAINTENANCE.  THE MAINTENANCE
RECORDS CHECKLIST CAN ALSO BE USED TO ASSIST IN THE PERFORMANCE
OF A TECHNICAL INSPECTION ON THE FORMS AND RECORDS BY QUALITY
CONTROL AND MAINTENANCE SUPERVISORY PERSONNEL.
       (8)  ALL DA FORMS 2408-13-1/2408-13-1-E,
2408-13-2/2408-13-2-E, AND DA FORM 2408-13-3 USED TO CORRECT
FAULTS OR DEFICIENCIES DISCOVERED DURING THE INSPECTION WILL
BECOME PART OF THE LOGBOOK FLIGHT PACK AND WILL BE NUMBERED AS
PAGES IN THE FLIGHT PACK.
       (9)  DA FORMS 2408-13-2/2408-13-2-E COMPLETED TO DOCUMENT
DISASSEMBLY OF AN INSPECTION TASK WILL BE ATTACHED TO THE
MAINTENANCE CHECKLIST.  THESE DA FORMS 2408-13-2/2408-13-2-E
WILL BE INSERTED IN THE CHECKLIST AND PAGE NUMBERED AS
SUPPLEMENTS TO THE CHECKLIST.
       (10)  ALL FORMS COMPLETED DURING THE INSPECTION WILL BE
MADE AVAILABLE TO QUALITY CONTROL AND TEST FLIGHT PERSONNEL FOR
A TECHNICAL INSPECTION PRIOR TO AIRCRAFT MAINTENANCE TEST
FLIGHT.  AFTER THE TEST PILOT REVIEWS ALL LOGBOOK AND RELATED
MAINTENANCE FORMS AND RECORDS, THE INSPECTION CHECKLIST WILL BE
RETAINED IN THE MAINTENANCE OFFICE.  AFTER COMPLETION OF THE
TEST FLIGHT, THE CLOSED OUT FLIGHT PACK WILL BE ATTACHED TO THE
MAINTENANCE CHECKLIST AND FILED IN THE SIX MONTH FILE PER
PARAGRAPH 2-9 OF DA PAMPHLET 738-751.
    D.  SPECIAL INSTRUCTIONS.  FORMS AND RECORDS PREPARATION
INSTRUCTIONS PRINTED IN THE CURRENT SCHEDULED MAINTENANCE
INSPECTION CHECKLIST WILL BE DISREGARDED AT THIS TIME AND THE
FOLLOWING INSTRUCTIONS WILL BE USED.
    E.  PREPARATION OF CHECKLIST.
       (1)  COMPLETE THE HEADING INFORMATION ON EACH PAGE OF THE
CHECKLIST.
       (2)  PERSONNEL PERFORMING MAINTENANCE INSPECTION TASKS,
OUTLINED IN THE INSPECTION CHECKLIST, WILL PLACE THEIR
SIGNATURES AND INITIALS ON THE SIGNATURE SHEET IN THE
CHECKLIST.  THE PURPOSE OF THE SIGNATURE SHEET IS TO PROVIDE A
CORRELATION BETWEEN INITIALS ENTERED ON THE INDIVIDUAL CHECKLIST
SHEETS AND THE ACTUAL NAMES OF THE MAINTENANCE PERSONNEL
ACCOMPLISHING INSPECTION REQUIREMENT TASKS.
       (3)  WHEN AIRCRAFT DISASSEMBLY IS REQUIRED BY THE
MAINTENANCE INSPECTION CHECKLIST TO COMPLETE AN INSPECTION
REQUIREMENT, ENTER THE MAINTENANCE RELATED ACTIONS ON A DA FORM
2408-13-2/2408-13-2-E.  THE NUMBER "2408-13-2" WILL BE ENTERED
IN THE "FAULT AND/OR REMARKS" BLOCK ON THE INSPECTION
CHECKLIST.  THIS WILL DENOTE THAT A DA FORM
2408-13-2/2408-13-2-E HAS BEEN INITIATED TO RECORD THE
DISASSEMBLY.  COMPLETE THE DA FORMS 2408-13-2/2408-13-2-E PER
THE INSTRUCTIONS GIVEN BELOW.
       (4)  UPON COMPLETION OF EACH INSPECTION REQUIREMENT TASK
LISTED IN THE CHECKLIST, IF NO FAULTS WERE FOUND RELATING TO THE
INSPECTION, THE PERSON WHO PERFORMED THE INSPECTION REQUIREMENT
TASK WILL ENTER "INSP OK" IN THE "ACTION TAKEN" BLOCK OF THE
CHECKLIST AND WILL ENTER THEIR INITIALS IN THE "INITIAL" BLOCK.
       (5)  WHEN A DEFICIENCY OR FAULT THAT IS RELATED TO THE
INSPECTION REQUIREMENT TASK IS DISCOVERED, ENTER "INSP COMPL" IN
THE "ACTION TAKEN" BLOCK OF THE CHECKLIST AND ENTER THE FAULT OR
DEFICIENCY ON DA FORM 2408-13-1/2408-13-1-E.  THE PERSON THAT
PERFORMED THE INSPECTION REQUIREMENT TASK WILL ENTER THEIR
INITIALS IN THE "INITIAL" BLOCK.
       (6)  MAINTENANCE OPERATIONAL CHECKS (MOC) AND MAINTENANCE
TEST FLIGHTS (MTF) WILL BE COMPLETED AND RECORDED PER DA
PAMPHLET 738-751 AND TM 1-1500-328-23.
    F.  DISPOSITION.
       (1)  THE COMPLETION OF A SCHEDULED MAINTENANCE INSPECTION
(I.E., INTERMEDIATE, PHASE, PERIODIC, COMBAT PHASE OR
PROGRESSIVE PHASED MAINTENANCE) INCLUDING THE MAINTENANCE TEST
FLIGHT, WILL BE RECORDED ON A DA FORM 2408-13-1/2408-13-1-E AND
DA FORM 2408-15/2408-15-E (HISTORICAL RECORD FOR AIRCRAFT).  THE
DA FORM 2408-13-1/2408-13-1-E IS LOCATED IN THE AIRCRAFT
LOGBOOK, WHILE THE DA FORM 2408-15/2408-15-E IS FILED IN THE
AIRCRAFT HISTORICAL RECORD FILE IN THE MAINTENANCE OR QUALITY
CONTROL OFFICE.
       (2)  AFTER COMPLETION OF THE SCHEDULED MAINTENANCE
INSPECTION, REQUIRED GENERAL TEST FLIGHT, AND AT THE END OF THE
MISSION DAY, THE COMPLETED PAPER WORK, INCLUDING DA FORMS
2408-13/2408-13-E, 2408-13-1/2408-13-1-E, 2408-13-2/2408-13-2-E,
2408-13-3 AND SCHEDULED MAINTENANCE CHECKLIST THAT DOCUMENTED
THE INSPECTION WILL BE FILED IN THE AIRCRAFT SIX MONTH FILE.
WHEN THESE FILES BECOME SEVEN MONTHS OLD THEY WILL BE PACKAGED
AND MAILED TO THE COMMANDER, CORPUS CHRISTI ARMY DEPOT, ATTN:
AMSAT-I-MEDA (STOP 55). CORPUS CHRISTI ARMY DEPOT 78419-6195.
    G.  PREPARATION INSTRUCTIONS (BY BLOCK TITLE) FOR COMPLETION

       (8)  FAULT.  ENTER A SHORT DESCRIPTION OF THE RELATED
MAINTENANCE ACTION, THAT NEEDS TO BE DONE, TO PROPERLY COMPLETE
